Try adjusting the Pushup for nipple depth to see if it helps, otherwise that just means the clothes are not compatible

No, it's not the card's problem. Rather, it has two versions: a paid version with a head model and a free clothing version without a head model. The MOD loader prioritises the free version, which is intentionally designed by the author. You just need to search for the MOD of that character card in the mods, find one with a similar name by the same author, and delete the smaller one. Generally, the smaller one is the version without the head model. If you don't delete it, the head model will always be missing.Thank you.
